Management Board of Centrum Medyczne ENEL-MED S.A. hereby presents a list of shareholders holding at least 5% of votes at the Ordinary General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. company that took place on 16 June 2014:

1.   Koremia Investments Ltd – voting entitlement of shares held: 7.124.000. Percentage share in the number of votes at the General Meeting convened on 16 June 2014: 37.75%. Percentage share in the total number of votes: 30.23%.

2.   Versinus Ltd – voting entitlement of shares held 7.123.950. Percentage share in the number of votes at the General Meeting convened on 16 June 2014: 37.75%. Percentage share in the total number of votes: 30.23%.

3.   Otwarty Fundusz Emerytalny PZU „Złota Jesień” (Open Pension Fund PZU „Złota Jesień”) – voting entitlement of shares held: 1.680.000. Percentage share in the number of votes at the General Meeting convened on 16 June 2014: 8.9%. Percentage share in the total number of votes: 7.13%.

4.   Smerona Holdings Ltd – voting entitlement of shares held: 992.800. Percentage share in the number of votes at the General Meeting convened on 16 June 2014: 5.26%. Percentage share in the total number of votes: 4.21%.

5.   Elvestra Ltd- voting entitlement of shares held 972.650. Percentage share in the number of votes at the General Meeting convened on 16 June 2014: 5.15%. Percentage share in the total number of votes: 4.13%.
