14/2015 List of shareholders holding at least 5% of votes at the Annual General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. held on 20 May 2015

The Management Board of Centrum Medyczne ENEL-MED Spółka Akcyjna (hereinafter: “Company”) would like to present the list of shareholders holding at least 5% of votes at the Annual General Meeting of Shareholders of the Company held on 20 May 2015.:

1.   Adam Stanisław Rozwadowski – number of votes arising from shares held: 7,124,000. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 20 May 2015: 33.37%. Percentage share in the total number of votes: 30.23%.

2.   Anna Maria Rozwadowska – number of votes arising from shares held: 7,123,950.Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 20 May 2015: 33.37%. Percentage share in the total number of votes: 30.23%.

3. Generali Otwarty Fundusz Emerytalny – number of votes arising from shares held: 2,370,000.Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 20 May 2015: 11.10%. Percentage share in the total number of votes: 10.06%.

4.   Otwarty Fundusz Emerytalny PZU “Złota Jesień” – number of votes arising from shares held: 1,778,000.Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 20 May 2015: 8.33%. Percentage share in the total number of votes: 7.54%.
