18/2013 Correction of the current report no. 18/2013: List of shareholders holding at least 18/2013 of votes at the Annual General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. held on 23 May 2013

The Management Board of Centrum Medyczne ENEL-MED Spółka Akcyjna, with its registered office in Warsaw, would like to present the correction of current report no. 18/2013 of 23 May 2013. In current report no. 18/2013, by mistake, the complete list of shareholders holding at least 5% of votes at the Annual General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. held on 23 May 2013 was not presented.

Due to the above, the Management Board of Centrum Medyczne ENEL-MED S.A. would like to correct the content of current report no. 18/2013 and below present the list of shareholders holding at least 5% of votes at the Annual General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. held on 23 May 2013.

1. Koremia Investments Ltd – number of votes arising from shares held: 7,124,000. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 23 May 2013: 38.73%. Percentage share in the total number of votes: 30.23%.

2. Versinus Ltd – number of votes arising from shares held: 7,123,950. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 23 May 2013: 38.73%. Percentage share in the total number of votes: 30.23%.

3. Otwarty Fundusz Emerytalny PZU „Złota Jesień” – number of votes arising from shares held: 1,200,000. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 23 May 2013: 6.52%. Percentage share in the total number of votes: 5.09%.

4. Smerona Holdings Ltd – number of votes arising from shares held: 992,800. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 23 May 2013: 5.4%. Percentage share in the total number of votes: 4.21%.

5. Elvestra Ltd – number of votes arising from shares held: 972,650. Percentage share in the number of votes at the Annual General Meeting of Shareholders convened for 23 May 2013: 5.29%. Percentage share in the total number of votes: 4.13%.

The aforementioned correction is a result of the obvious clerical error. Since the mistake was identified, the Company’s Management Board has decided to immediately correct the report in question. The remaining part of the report is unchanged.
