15/2013 Extension of cooperation with UNIQA Towarzystwo Ubezpieczeń S.A.

The Management Board of Centrum Medyczne ENEL-MED S.A. would like to inform you that on 22 April 2013, the agreement with UNIQA Towarzystwo Ubezpieczeń S.A. (“UNIQA”) was signed.

The subject of the agreements is the provision of medical services to employees, contractors and their families. The services will be provided at centres of Centrum Medyczne ENEL-MED and at other medical centres cooperating with the Company.

The agreement was concluded for indefinite period starting 1 May 2013, and its value depends on the number of persons covered by the pre-paid medical plan in a given month, and might reach about PLN 3,200,000.00 in five years.

Centrum Medyczne ENEL-MED has been cooperating with UNIQA Towarzystwo Ubezpieczeń for four years. Until now, the cooperation included the provision of medical services to customers of the insurer, who purchased insurance policies covering medical services. Starting 1 May 2013, the cooperation will be extended to the provision of medical services to employees, contractors and their families.
