21/2013 Granting a commercial power of attorney

The Management Board of Centrum Medyczne ENEL-MED S.A. with its registered office in Warsaw announces that on 05 June 2013 it granted Ms Beata Irena Rozwadowska a joint commercial power of attorney that includes the power to perform such acts and activities, whether in front of a court of law or out of court, as may be connected with the pursuit of the company’s business, jointly with a member of the Management Board or another holder of the company’s commercial power of attorney. From the date of granting the power of attorney, Ms Beata Rozwadowska may receive statements and/or letters made or addressed to the company.

Beata Rozwadowska graduated from University of Warsaw (Applied Linguistics). In 1992-1995, she worked as a language teacher and translator, and was involved in the PHARE programme at the Polish Ministry of Labour and Social Policy. Later, Beata Rozwadowska ran a training centre for medical and dental practitioners. Since 1996, she worked at Centrum Medyczne ENEL-MED S.A, where – as Sales & Customer Service Manager – she implemented the Enel-Care subscription programme. In 1999, she completed a postgraduate degree in Market & Insurance in Healthcare at the Higher School of Banking, Finance and Management in Warsaw. Since 2008, Beata Rozwadowska has been the Quality Management System representative of the Management Board of Centrum Medyczne ENEL-MED S.A.

Beata Rozwadowska is not listed in the Register of Insolvent Debtors maintained under the Polish Act on the National Court Register. She is not engaged in any work or activity other than at Centrum Medyczne ENEL-MED S.A., nor is she a shareholder or partner in any company or partnership. Beata Rozwadowska is not a member of any governing body of any company or any other legal person which may be a competitor to Centrum Medyczne ENEL-MED S.A.
