The Management Board of Centrum Medyczne ENEL-MED S.A. would like to present the list of shareholders holding at least 5% of votes at the Extraordinary General Meeting of Shareholders of Centrum Medyczne ENEL-MED S.A. held on 2 October 2013:

1. Koremia Investments Ltd – number of votes arising from shares held: 7,124,000.Percentage share in the number of votes at the General Meeting of Shareholders convened for 02 October 2013: 33.97%. Percentage share in the total number of votes: 30.23%.

2. Versinus Ltd – number of votes arising from shares held: 7,123,950.Percentage share in the number of votes at the General Meeting of Shareholders convened for 02 October 2013: 33.97%. Percentage share in the total number of votes: 30.23%.

3. Generali Otwarty Fundusz Emerytalny – number of votes arising from shares held: 2,377,000.Percentage share in the number of votes at the General Meeting of Shareholders convened for 02 October 2013: 11.34%. Percentage share in the total number of votes: 10.09%.

4. Otwarty Fundusz Emerytalny PZU “Złota Jesień” – number of votes arising from shares held: 1,400,000.Percentage share in the number of votes at the General Meeting of Shareholders convened for 02 October 2013: 6.68%. Percentage share in the total number of votes: 5.94%.
