5/2012 Conclusion of an annex to the agreement with the National Health Fund – Subcarpathian Voivodeship Branch in Rzeszów for provision of health care services at the diagnostic laboratory in Mielec

The Management Board of Centrum Medyczne ENEL-MED S.A. (the Company) announces that, on 23 January 2012, it was notified that the National Health Fund – Subcarpathian Voivodeship Branch in Rzeszów had signed an annex to the Agreement of 31 December 2010 for provision of health care services in the form of specialist outpatient care (magnetic resonance imaging examinations) at the diagnostic laboratory in Mielec, located in the building of the E. Biernacki District Hospital in Mielec at Żeromskiego 22.

The annex increases the value of the agreement of 31 December 2010 from the amount of PLN 891.187,60 (in the settlement period beginning on 1 January 2011 and ending on 31 December 2011) to the amount of PLN 1.048.424,00 in the settlement period beginning on 01 January 2012 and ending on 31 December 2012.

Other terms of the agreement do not change.

The conclusion of this annex begins the fifth year of cooperation between Centrum Medyczne ENEL MED S.A. and the Subcarpathian Voivodeship Branch in Rzeszów as regards provision of services in the field of magnetic resonance in Mielec.
